General ATAPS Services
GPGC has funding for general ATAPS services until June 2011. Eligible patients are youth up to 25 years, A&TSI, disadvantaged (HCC Holders). To reduce administration costs and monitor the funding closely we have changed the service delivery model in the following ways:
- Reduced number of providers
- No referral pads, see below for referral procedures
- Feedback loop from providers
- GPs and AH Providers are asked to comply with the service delivery requirements highlighted in the Guidelines
- Gain patients consent and provide them with a brochure

Word Document Referral - Complete template and send via Medical Objects or fax to your chosen allied health professional via the method outlined next to their details. These must be sent to AH not given to patient.
- Send the referral to Elaine Crawt (MO address) via Medical Objects or fax 07 5507 7700
- Once the AH provider has received the referral they will contact the patient to commence treatment
- The AH provider will send you a feedback form to ensure you are aware of patient progress
- Each set of six sessions will require a referral form

This service is available at the Lakeside Rooms, Robina and at headspace Gold Coast for 12-25 year olds. Please use the referral forms below:

Perinatal Depression Group Programs
This program is for women diagnosed with perinatal depression. There are 3 locations for the group programs; Southport, Robina and Tweed/Banora area, please contact Elaine or Erin for further details on 07 5507 7777 ext 2.
